Adults and children from 12 years of age, weighing 40-80 kg: -20 mg/kg/day (equivalent to 80 mg/kg given orally twice daily for 7 days), 50 mg/kg/day (equivalent to 200 mg/kg given intravenously) and 100 mg/kg/day (equivalent to 400 mg/kg given orally twice daily for 7 days).
-20 mg/kg/day (equivalent to 20 mg/kg given intravenously) and -40 mg/kg/day (equivalent to 40 mg/kg given orally) should be used instead of the 200 mg/kg/day given intravenously. -40 mg/kg/day (equivalent to 400 mg/kg given orally) and the 100 mg/kg/day should be used instead of the 200 mg/kg/day given intravenously.
